Delivering Medication Therapy Management Services in the Community

In a collaborative effort to expand Medication Therapy Management (MTM) services, advance pharmacy practice in the community and long-term care settings, and optimize medication use to improve the health of the public in Maryland, the Maryland Chapter of the American Society of Consultant Pharmacists (MD-ASCP), the Maryland Pharmacists Association (MPhA), and the University of Maryland School of Pharmacy (UM-SOP), present:

A Certificate Training Program for Pharmacists developed by the American Pharmacists Association (APhA) and the Association of Consultant Pharmacists (ASCP), is supported in part by educational grants from: Boehringer Ingelheim, Eisai, Lilly, Janssen, Ortho-McNeil Janssen, sanofi-aventis, and Wyeth.

Successful completion of the self-study learning component will result in 10 hours of continuing pharmacy education (1 CEU) – ACPE UPN: #202-203-07-089-H04. An additional 11 hours of continuing pharmacy education (1.1 CEU) – ACPE UPN: #202-203-07-088-L04 – will be granted for completing the live training seminar. CPN - 202-0012.
