Deciding on a Plan of Study

Plan of Study Form -  Class of 2011 & 2010  Class of 2009
Use this form to develop your plan of study. You must submit an updated form each semester, signed by your advisor, to the Student Affairs Office in Pharmacy Hall, room 722.
