These forms ensure that we have all the information necessary to complete requests in a timely fashion. They are in PDF format, and can be filled in with Adobe Acrobat and either submitted electronically or printed out.

• Student Organization Mailing List Creation/Renewal
  Use this form to request a new mailing list (listserv) for your student organization, or renew an existing one. Student lists must be renewed each year.
   
• Student Organization Web Site Creation/Renewal
  Use this form to request access to an existing student organization Web site, or to request that one be set up. Web sites do not need to be renewed yearly per se, but student access will be revoked at the end of each academic year unless it is renewed.
   
• Video Digitization
  Use this form to request that part of a DVD or video tape be digitized for use in PowerPoint or another computer program. Please note that you must have permission from the copyright holder of the material you wish to digitize.
